RSTC Medical Form
All operators will require you to complete a medical questionnaire before any in-water activities. Please review this example, and if you answer "Yes" to any questions, a medical doctor must confirm and sign off that you are fit to dive.
This is an example of a statement outlining the standards expected of you and others to ensure safe diving and enhance your comfort. All operators will require you to read and sign this statement, acknowledging your understanding and agreement, before providing any dive services.
